The Application Form is a 2-page pdf form that you can fill out on your computer, and then either
a. print and mail it to the address at the end of the form, or
b. save the completed pdf file and email it to ruellafrank (at) somaticstudies.com

Depending on the browser you’re using (IE, Firefox, Safari or Chrome), clicking this link will either open the form in a new tab or window in your browser, or download it to your computer. The preview below also links to the pdf.

If the pdf opens right away in your browser, you can go ahead and fill it out, then print or save the file (or both, if you like).
